The results suggest that the aggrecan gene VNTR polymorphism contributes to the pathogenesis of intervertebral disc degeneration. The presence of the most common allele (allele with 26 repeats) predisposes the individuals for an accelerated degenerative process.

The biomechanical behavior of a new interspinous implant was assessed using a combined in vitro and finite-element analysis. The effect of this implant appeared mainly in flexion-extension: the in vitro experiment underlined a motion reduction and the finite-element analysis showed a stress reduction in disc fibers and anulus matrix.

This retrospective comparative study analyzed the functional and radiographic results after anterior screw fixation of odontoid fractures, with the particular interest to compare the outcome between younger and elderly patients. The results of this series revealed an encouraging outcome in both treatment groups and younger patients to have a superior outcome regarding fracture healing as well as morbidity and mortality.

Fourteen cases of severe neurologic deficits following vertebroplasty and kyphoplasty are retrospectively described. This devastating complication may develop acutely or in a delayed fashion. Delayed onset of injury may be heralded by an insidious onset of back pain. Potential methods to minimize the risk of injury are discussed.

The 2007 Cochrane Review presents an analysis of outcome data from 40 randomized and 2 quasi-randomized controlled trials of surgical interventions used in the treatment of lumbar disc prolapse. Seventeen of the trials have been published in the last 8 years. Meta-analysis allows conclusions to be drawn on the effectiveness of some interventions.

The present genetic association study comprehensively investigated variations of melatonin receptor 1B gene in AIS patients. An association was found between a promoter polymorphism of melatonin receptor 1B gene and AIS, which suggested that melatonin receptor 1B gene was a disease predisposition gene.

Trends in epidural steroid injection use using Medicare claims data were evaluated. Lumbosacral injection use and costs increased dramatically from 1994 to 2001. Less than half were performed for radiculopathy, where the greatest evidence of benefit is available. These findings suggest a lack of consensus regarding the indications for ESIs and are cause for concern given the costs.

In Spanish patients, the influence of FAB on disability and quality of life is irrelevant and much lower than that of pain severity. However, FAB influence LBP-related sick leave. Each additional point on the FABQ baseline score increases by 2.4% the odds of being on sick leave for up to 60 days during the following year, and by 7.7% the odds of being sick listed for 61 days or more. This effect of FAB is not being confounded by other variables.

The SQLI score distribution was examined in age, management, curve severity, and curve type subgroups of females with AIS. Most ceiling effects were observed in younger subjects with curves below 50° Cobb angle or under observation and in the back pain and satisfaction domains. The SQLI score distribution may limit its discriminative ability.

A systematic review of the surgical management of degenerative spondylolisthesis. Fusion lead to a better clinical outcome than decompression alone. The use of supplementary instrumentation significantly increased the probability of achieving solid fusion but no significant change in clinical outcome occurred.

Porcine lumbar anterior longitudinal ligaments are examined for temperature dependence at fast strain rates. The ligaments were strained to 20% of their initial length and tested in the temperature range of 37.8°C to 4.4°C. The ligaments were found to be temperature dependent, and a viscoelastic model was developed.

Dorsal arthrodesis of the thoracic spine was performed in prepubertal rabbits to evaluate by periodical CT scan the changes in thoracic growth patterns. The dorsoventral thoracic diameter grows more slowly than the laterolateral thoracic diameter. The sternum, the total dorsal, and total ventral lengths of the thoracic vertebral bodies as well grow less. The crankshaft phenomenon is evident at the fused vertebral levels where there is a reduction of thoracic kyphosis.

This prospective cohort study assessed the reliability and diagnostic value of the vertebral wedge ratio (WR) to the segmental Cobb angle (CA) in individuals with Scheuermann's kyphosis, postural roundback, and in normal subjects. Although the CA method presented high intraobserver and interobserver reliabilities, the vertebral WR is a good reliable method in assessing sagittal spine deformities. Moreover, the apex vertebral WR may be more sensitive to the radiographic characterization of Scheuermann's kyphosis. Various diagnostic measurement parameters of sagittal spine deformities are proposed.

The influence of pain-related fear on lumbar excursions of individuals with subacute back pain is assessed using a target reaching paradigm. Participants with high pain-related fear restrict lumbar motion more than participants with low pain-related fear. A case is reported about atypical syringomyelia with Chiari malformation in MR imaging, which showed enlarged spinal cords and gray matter T2 prolongation without cavitation in association with continued cerebrospinal fluid obstruction in upper spine, and which was reversible quickly after restoration of patency of cerebrospinal fluid pathways. This atypical syringomyelia is a kind of presyringomyelia state.

E471 Horner Syndrome due to a Solitary Osteochondroma of C7: A Case Report and Review of the Literature.

A 23-year-old woman with osteochondroma of the lower cervical spine who presented with Horner syndrome was reported. The patient demonstrated a complete disappearance of clinical symptoms on the follow-up examination 60 days after surgery. This case report supports surgical intervention of symptomatic osteochondroma of the cervical spine.
