This study evaluated the effects of two chair designs on sewing machine operators with neck/shoulder pain. The chair with a curved seat pan was associated with a significant decline in neck/shoulder pain compared with the control group. The findings may be relevant to workers who perform visually demanding manufacturing tasks.

The Efficiency of Gabapentin Therapy in Patients With Lumbar Spinal Stenosis.

Fifty-five patients with lumbar spinal stenosis were randomized to receive oral gabapentin or nothing along with the standard treatment. Patients receiving gabapentin showed superior reduction in pain scores and increased their walking distance when compared with control patients. Gabapentin should be considered in the treatment of lumbar spinal stenosis.

Peripheral nerve segments subjected to sustained mechanical stimulation results in massive proliferation of Schwann cells. Transplantation of these compressed nerve segments to rat hemisected spinal cord model results in improved functional outcome with superior axonal regeneration.

We correlated changes in SSEP and NAP with behavior and pathology in a preganglionic and postganglionic radiculopathy model. SSEP was sensitive but not specific to preganglionic and postganglionic radiculopathy. NAP, however, showed the specificity of reduced amplitude and conductive velocity with postganglionic radiculopathy but remained normal with preganglionic lesioning.

Hybrid Testing of Lumbar CHARITE Discs Versus Fusions.

One- and 2-level CHARITÉ artificial discs were compared with corresponding simulated fusions in flexion-extension and bilateral torsion using hybrid testing protocol with follower load. CHARITÉ discs preserved motions, while fusions resulted in increased motions at all available free levels.

Development and Validation of a Finite Element Model of the Occipito-Atlantoaxial Complex Under Physiologic Loads.

A detailed finite element model of occipito-atlantoaxial complex was developed. It is in good agreement with experimental data (both the range of motion and neutral zone) under the different loading conditions. The ligaments show their general laxity in the physiologic range.

Utility of Flexion-Extension Radiographs in Evaluating the Degenerative Cervical Spine.

We reviewed the neutral and dynamic lateral cervical radiographs of 206 degenerative patients. There were no findings on dynamic radiographs that impacted the initial evaluation or treatment. We no longer recommend dynamic cervical radiographs as a screening tool in the initial evaluation of the degenerative population.

Clinical Classification of Patients With Lumbar Spinal Stenosis Based on Their Leg Pain Syndrome: Its Correlation With 2-Year Surgical Outcome.

A total of 174 patients who underwent surgery for lumbar spinal stenosis were classified based on the type of leg pain. The preoperative leg pain type predicted 2-year outcomes. After surgery, patients with unilateral leg pain had greater improvement in function and leg symptoms than patients with bilateral leg pain.

Minimum 5-Year Follow-up Surgical Results of Post-traumatic Thoracic and Lumbar Kyphosis Treated With Anterior Instrumentation: Comparison of Anterior Plate and Dual Rod Systems.

In this study, the effects of anterior decompression strut grafting, anterior instrumentation on sagittal and frontal plane deformities have been investigated in 40 patients with post-traumatic kyphosis. The type of the instrumentation system did not differ significantly in terms of kyphotic deformity correction rates and correction losses and PFA scores after 5-year follow-up.

Clinical Outcome of Symptomatic Unilateral Stress Injuries of the Lumbar Pars Interarticularis.

A total of 42 patients with unilateral lumbar pars stress injuries or spondylolysis were treated conservatively for 6 months; 34 of 42 patients had a good functional outcome following rest and antilordotic bracing. Eight patients who did not respond to nonoperative treatment underwent direct repair. Seven of these 8 active sporting individuals had significant improvement in symptoms and returned to active sports after a mean of 7 months following surgery. One patient with spina bifida at L5 with a right-sided pars defect remained symptomatic after direct repair of the defect.

Revision and Explantation Strategies Involving the CHARITE Lumbar Artificial Disc Replacement.

This study reviews the results from 20 consecutive anterior revision procedures in patients with complications following lumbar total disc replacement with the CHARITÉ artificial disc. Implant revision was successful in all cases. When necessary, anterior revision surgery can be performed safely when a strategic approach is used.

A retrospective analysis was conducted to identify differences in clinical and radiologic outcomes between the CHARITÉ and ProDisc disc prostheses. Both devices were effective in relieving back pain and disability of the patients, but high incidences of facet degeneration of the level of replacement and disc degeneration at the adjacent segment were identified.

Quantitative Computed Tomography-Based Predictions of Vertebral Strength in Anterior Bending.

The ability of various QCT-based measures to predict vertebral strength in anterior bending was evaluated. Bone mineral density, axial and bending rigidity measures, and finite element models were weakly correlated with strength, which suggests that these QCT-based measures are unable to predict failure under physiologic loading conditions.

This study examined associations of leisure-time physical activity and sedentary activities with neck/occipital and shoulder pains in a large birth cohort of adolescents aged 15-16 years. Psychiatric disorders in patients with chronic disabling occupational spinal disorders are much more likely to develop after the onset of the work injury, rather than be preexisting. The results of the study indicate that such injuries and accompanying stressors associated with chronicity are likely to be precipitants, rather than consequences, of psychiatric disturbance.

Treatment of Vertebral Osteomyelitis by Radical Debridement and Stabilization Using Titanium Mesh Cages.

Eighty-eight cases of vertebral osteomyelitis were treated by radical debridement and stabilization using titanium mesh cages. At latest follow-up, all patients showed a solid bony fusion without any recurrence of infection. Titanium mesh cages effectively reconstructed anterior column stability and restored the sagittal profile.

The authors analyzed data from a consensus panel in 4 ways to assess what influenced the choice of priorities for interventions to prevent back disability. The main influences related to different views about disability and other elements of personal experience. Secondary influences related to the prioritization process itself. There was a weak association between background and affiliation, and particular choices or views about disability.

Reported here is the case of a 42-year-old man involved in a motorcycle accident in which the brake caliper penetrated the lumbar region and entered the lumbar canal. The patient was taken urgently to the operating room and underwent removal of the foreign object. The patient made a good neurologic recovery.

Total En Bloc Spondylectomy of C5 Vertebra for Chordoma.

A chordoma, confined to the vertebral body of C5, was excised by total en bloc spondylectomy via staged posterior and anterior approaches. No neurovascular structures were killed, and wide surgical margins were achieved. The patient received postoperative adjuvant proton beam radiation therapy and remains disease-free 9 years after the operation.

Lumbar Synovial Joint Hematoma in a Patient on Anticoagulation Treatment.

A 67-year-old man on anticoagulation therapy presented with progressive neurologic symptoms. He was proven to have a synovial joint hematoma macroscopically and microscopically. This is the first report of a synovial cyst hematoma due to anticoagulation therapy. Spinal decompression proved to be the definitive treatment.
