求助如何回复审稿者这问题

我联合cTcT与SOFA预测肺栓塞的死亡诊断模型，审稿者意见如下：It’s unclear how the authors determine the threshold for two reference models based respectively on cTcT and SOFA (aka, how to balance the true positive and false positive)，这个问题是什么意思？问预值怎么确定？真阳性和假阳性，真阳性不就是敏感性，假阳性不就1减去敏感性吗？求大侠支个怎么回答这问题？非常感谢！！
