An 18-year-old female presents with a 3-day history of progressive weakness and numbness of her legs, urinary retention and back pain some 2 weeks following an upper respiratory infection. On examination there is spastic paraparesis, pinprick sensory level up to T5, impaired vibration sense to her knees and extensor plantars. Examination of her cranial nerves and upper limbs is normal. MRI of the spine is normal. CSF analysis reveals 50 cells/mm , over 90% lymphocytes with normal protein and glucose levels and negative oligoclonal bands.
