请大神们帮忙看看这啥意思？各种不懂！

投稿一篇SCI杂志，文章被接收了，编辑回了封信，里面提到了这些话

Your manuscript will be typeset and copy-edited for publication. An e-mail with your electronic proofs will be sent to you within two weeks of receipt of all required items. 

Please be sure that you have forwarded the following items to the Editorial Office (apsurg@tds.net). Your article cannot begin the production process until these items have been completed.

1) Written permission from any patient whose recognizable face is included in the figures
2) Permission forms for all reprinted material (if applicable to your paper)
3) Please confirm the email address and telephone number for the corresponding author and an alternate email address (either for the corresponding author, or for a co-author) in the event we do not receive a response to the page proofs in a timely manner.

第一条明白，第二第三条是个啥意思？第二要Permission forms，根本没有这个表格啊，第三条根本看不懂
