Uniportal VATS resection through a small incision to remove a residual mediastinal mass. The patient had a previous cervicotomy to remove a goiter located on the neck. During the follow up the residual goiter grown down towards the thoracic cavity so an uniportal VATS approach was decided. The right laryngeal nerve was carefully dissected, exposed and preserved. The postoperative course of the patient was uneventful
