A 68-year old female smoker with dyslipidemia, in a routine check (asymptomatic), negative T waves were detected in V1 to V3. A treadmill test was performed. No cardiac symptoms were provoked, and the ECG response to effort was not typical for coronary artery disease.

1.It is a normal variant in women and no further evaluation is needed.

2.It is a technical error.

3.Although the T-wave negativity persists, it is associated with a ST-segment depression and additional diagnostic tests are needed.
