Postintensive care syndrome 

In addition to preexisting psychiatric comorbidity, critically ill patients also have high risk of posthospitalization cognitive morbidity. 30–80% of ICU survivors experience cognitive impairment, and the major risk factor is delirium [29]. Although light has been shed on this topic since the Brussels Roundtable on surviving intensive care in 2002 [30], there is still a lot unknown about post-ICU cognitive impairment. PICS is defined as the development of cognitive, psychological, and/or physical disability after critical illness requiring ICU care [31]. In 2013, PICS was recognized by the SCCM Council as an ‘emerging condition that deserved intense focus’ [32]. Cognitive impairment, depression, anxiety, and PTSD can persist for years in patients who have survived critical illness and there is no widely established rehabilitation plan. This phenomenon has led to the increasing establishment of ICU survivor clinics, whose benefits need further study [33 &]. One of the first ICU survivor clinics in the United States, the Critical Care Recovery Center (CCRC), was developed using the concept of ‘agile implementation:’ a reproducible and scalable process to rapidly localize, implement, and sustain evidence-based healthcare services’ [34 &]. This process relied on ‘implementation sprints’ [34 &], which is focused work aimed to address unforeseen issues that arise while attempting to implement protocols in the local setting. The Agile Implementation Playbook was key in rapidly implementing evidence-based practices using a collaborative care model developed for older adults [34 &,35]. This novel method may be applicable to other areas of the country looking to implement ICU survivor clinics. Unfortunately, under-recognition of PICS continues to be a problem. The same team that started the CCRC has developed and adapted a screening tool, the Healthy Aging Brain Care Monitor (HABC). This selfreported assessment is a 27-item questionnaire that can be administered in person, via telephone, or on the internet. It has been validated with standard tools for neuropsychological assessment, and with further study, may be utilized in all outpatient follow-up settings for ICU survivors [36 &].
