For decades, autoanalyzers have provided clinicians with a panel of basic serum chemistry results, including sodium, potassium, chloride, total CO2, glucose, blood urea nitrogen and creatinine. Although urine chemistries can also be determined with an autoanalyzer, urine tests are typically ordered piecemeal. This inexplicable practice seriously hampers our ability to thoroughly evaluate a variety of disturbances.
Urine chemistries are extremely valuable in the evaluation and management of patients with fluid and electrolyte or acid– base disturbances [1]. To avoid missing needed data, these should routinely include sodium, potassium, chloride, glucose, urea nitrogen and creatinine. It is most efficient to obtain all of these tests in a standardized basic urine metabolic panel obtained concurrently with the serum chemistry panel, with both panels measured by autoanalyzer.
