If HIT is suspected, all heparin containing products should be immediately stopped. Suitable anticoagulation should be initiated. Initially, the direct thrombin inhibitors should be used (e.g., argatroban and bivalirudin). Eventually, patients can usually be transitioned to warfarin. Direct FXa inhibitors have been used off label.
