A 76-year-old man presented with a two month history of emesis after meals, anemia, and a 25-pound weight loss over the past year. He had had vagotomy, antrectomy and Billroth II gastrojejunostomy for bleeding ulcer 34 years previously. Endoscopy revealed a nearly circumferential exophytic lesion at the gastrojejunostomy. Biopsy showed adenocarcinoma.

CT scan showed the gastric lesion, and no obvious nodal or metastatic disease.

The patient was taken to the operating room for attempted resection. Adhesions were divided to expose the gastrojejunostomy.
