颈椎病

1. 神经根性：颈肩痛，短期加重，上肢放射 pain radiation to upper liumbs, 皮肤麻木 paraesthesia，过敏，肌力下降 weakness，手指不灵活。Eaton and Spurling +
2. 脊椎型：最严重：四肢感觉，运动，反射，二便功能障碍综合征。有感觉障碍平面，Hoffmann and Babinski+
3. 椎动脉型：头晕，恶心，耳鸣，偏头痛，因周围大量交感神经节后纤维：诱发心悸，心律失常，胃肠道功能紊乱
4. 交感型：长期低头伏案有关，交感抑制或兴奋症状，症状多体征少，头晕头痛，颈痛，面部/躯干发麻，痛觉迟钝，心悸，耳鸣，记忆力/听力减退
腰椎间盘突出症
分类：（朋友突然脱离）（纤维环，后纵韧带的破裂，髓核游离）
1. 膨出型 Protrusion, posteriorly bulging disc, annulus intact
2. 突出型 Prolapse
3. 脱出型 Extrusion / rupture, fibrocartilaginous disc material extruded posterioirly, bulges to either side post. longitudinal lig.
4. 游离型 Sequestration / part of the nucleus lie freely in spinal canal or into intervertevral foramen.
5. Schmorl结节：指髓核经上下软骨板的发育性或后天性裂隙突入椎体骨内
6.经骨突出型：髓核沿椎体软骨终板和椎体之间的血管通道向前纵韧带方向突出
1. 症状
1. 腰痛：椎间盘突出刺激了外层纤维环及后纵韧带中的窦椎神经纤维, acute back pain
2. 坐骨神经痛：多为逐渐发生，疼痛为放射性，由臀部、大腿后外侧、小腿外侧至足跟部或足背, irritate the dural of adjacent nerve root, causing pain in the buttock, post. thigh and calf (sciatica),
3. 马尾综合征：中央型的腰间盘突出压迫马尾神经，出现大小便障碍，鞍区感觉异常 Cauda equina compression, causing urinary retention and perineal numbness.
2. 体征
1. 腰椎侧凸：为减轻疼痛的姿势性代偿畸形
2. 腰部活动受限：以前屈受限最明显, all back movements are restricted and forward flexion affected the most.
3. 压痛及骶棘肌痉挛：病变间隙的棘突间有压痛，按压椎旁1cm 处又沿坐骨神经的放射痛，约1/3 病人有腰部骶棘肌痉挛，使腰部固定于强迫体位 tenderness in the midline of the lower back, and paravertebral muscle spasm.
4. 直腿抬高试验及加强试验阳性 straight leg raising is restricted and painful of affected side,
5. 神经系统表现：感觉异常、肌力下降、反射异常 deminmished knee reflex and weakness, paraesthesia and numbness.
鉴别诊断：
1. 腰肌劳损 back-ache：中年，劳动姿势有关，休息可缓解，体检：直腿抬高阴性，下肢无神经受累
2. 腰椎管狭窄症：出现神经源性间歇性跛行，症状多，阳性体征少，MRI可明确诊断
3. 梨状肌综合征：臀部和下肢疼痛，休息缓解，体检：臀部肾牙痛，直腿抬高+，神经定位不明确，髋关节外展，外旋抗阻力可诱发。
4. 第三腰椎横突综合征
5.腰椎滑脱与椎弓峡部裂
6. 腰椎结核
7. 脊柱肿瘤 Vertebral tumours
8. 椎管内肿瘤
9. 盆腔疾病
10. 下肢血管病变
