Patients with severe chronic pain demonstrating significant side effects from oral, transdermal, or intravenous opioids with dose titration can be considered for intrathecal therapy. Those who do not achieve adequate analgesia despite increased doses of opioids should also be considered. The 2017 Polyanalgesic Consensus Conference (PACC) guidelines include a variety of disorders (see Tables 25.1 and 25.2).
