Venous admixture is calculated from the shunt equation, which assumes that the oxygen content of arterial blood equals the sum of content in the deoxygenated blood passing through the shunt and ‘ideally’ oxygenated blood passing through lung. To solve the equation, mixed venous oxygen content needs to be measured or assumed, and ideal alveolar Po2 calculated and converted into the oxygen content of pulmonary endcapillary blood. Physiological shunt is normally only a few percent of total cardiac output. The effect of varying levels of shunt on arterial oxygenation depends on the source of the shunt, but administration of oxygen to a patient will only correct the shunt caused by areas of lung with a V˙/Q˙ ratio less than 1 but greater than 0.
