I devoted a section to my book, Dry Eye Disease Treatment in the Year 2020, to conjunctival chalasis, a swelling of the conjunctiva due to repeated inflammation. We often see conjunctival chalasis in patients that have had years of Dry Eye Disease, DED. DED is an inflammatory condition that produces several inflammatory mediators that settle on the eye causing damage to the conjunctiva. Eventually the conjunctiva becomes redundant and overhangs on to the lid margin. I have seen where this worsens DED because the patient cannot get normal excretion from the Meibomian Glands when blinking. These glands sit at the lid margin and with each blink secrete the contents of the gland that make up the fatty layer of our tear film. In conjunctival chalasis overhanging conjunctiva blocks the glands preventing normal excretion. Sometimes after you treat the reason for DED and decrease inflammation the conjunctiva can come back to its normal position but sometimes even with improvement of the DED the conjunctiva retains its redundant form and we have to perform a surgical procedure to remove the excess conjunctiva.

I have tried several techniques like grafts, sutures, and glue but all these techniques have downsides. One thing that all these techniques have in common is patients report having pain after surgery that can last up to two weeks. The technique I employ here gives me the best functional and cosmetic effect. Patients love the fact that it is not painful during the procedure or after. These surgeries have improved Dry Eye Disease symptoms in patients with unresolved conj chalsis.
