Quantification of proteinuria should be done with a 24-hr urine collection with evaluation of urinary creatinine excretion to document validity of the collection. Spot collections have not been validated for heavy proteinuria or patients with rapidly changing creatinine values. A “spot” urine protein-to-creatinine ratio taken from the 24-hr collection defines the relationship between spot and “true” collections to monitor response to therapy.
