Question:What is the evidence that stimulant laxatives cause long-term bowel problems?
No clear-cut evidence exists for long-term bowel problems related to stimulant laxative use.
In the early 1960s and 1970s, "cathartic colon" was described in patients at barium enema examination as a dilated, flaccid-appearing colon that was associated with chronic constipation and long-term stimulant laxative use. This condition was assumed to be a consequence of damage to the myenteric plexus of the colon from the laxatives.[1-3] Early studies included additional histologic evaluation of colectomy specimens of patients who had used stimulant laxatives for 30-40 years and eventually received a colectomy because of uncontrolled, continuing constipation.[4] However, the underlying colonic disorder that prompted long-term laxative use was not well described, and many of these patients had begun stimulant laxative use prior to 1960.[5]
Early animal studies also suggested that colonic injury could occur from laxatives.[2] However, there were inconsistencies in these studies with respect to associated myenteric injury,[6] and further animal studies could not corroborate colonic findings when purified laxative extracts were used.[7] Later studies in humans and animals provide little evidence for sustained laxative-induced injury to colonic innervation.[8-10]
It is possible that early animal studies used laxative compound extracts containing other toxic materials that produced myenteric injury.[5] In addition, idiopathic constipation may be associated with myenteric plexus alteration in the absence of laxative use,[11] which suggests that some of these colonic specimens may have come from patients with primary colonic disorders. At this time, it seems unlikely that long-term stimulant laxative use will result in cathartic colon.[5]
