NEW YORK (Reuters Health) Jun 04 - Among Asian patients with vascular risk factors, middle cerebral artery (MCA) stenosis is frequently present, even in the absence of a history of stroke or transient ischemic attack, investigators report in the June issue of Neurology.
According to lead author Dr. Ka Sing Wong from the Chinese University of Hong Kong and colleagues, the most frequent lesion found in Asian patients with stroke is MCA stenosis, but little attention has been paid to asymptomatic MCA stenosis.
They therefore conducted a study in which 3057 clinic patients treated in Hong Kong (average age 56 years, 41% men) were assessed by transcranial Doppler. None of the subjects had a history of previous stroke or transient ischemic attack, but did have at least one vascular risk factor.
Altogether, 12.6% of patients had MCA stenosis.
According to multivariate analysis, statistically significant risk factors for MCA stenosis were age > 65 years, hypertension, diabetes, hyperlipemia (LDL cholesterol > 4.1 mmol/L), and ischemic heart disease.
"The prevalence (of MCA stenosis) escalated quadratically with increasing number of associated factors: from 7.2% for one, to 29.6% for four," Dr. Wong's team writes.
Surprisingly, cigarette smoking was not associated with risk, and body mass index > 23 appeared to be protective. The investigators suggest that inclusion of former smokers together with current smokers may have masked the risk. They theorize that the inverse association between obesity and intracranial atherosclerosis could be due to the presence of occult malignancy, underlying infection or inflammation, or malnutrition.
Why intracranial atherosclerosis is a common cause of stroke in Asian patients but not in white patients "deserves further investigation," Dr. Wong and colleagues maintain. They theorize that, since diabetes, hyperlipidemia and hypertension are common factors in both populations, differences in diet or genetics may be involved.
