The bacterium Bordetella pertussis is very contagious, and it's making a comeback. In April of this year, whooping cough was declared epidemic in Washington State. The CDC has already reported more than 17,000 cases nationwide this year -- more than twice as many as were reported at this time last year. In addition, 10 pertussis-related deaths have been reported.
Pertussis typically does seem to come in waves, but this year may be a tsunami. If nothing is done, the number of cases could set a record and this year could be the worst for pertussis in half a century.
Most deaths this year are in babies under 3 months old; infants and young children suffer the most. The last 10 years have seen a surge in pertussis-related deaths in infants. Household members are the main source of infection in more than 80% of cases. Why? The effect of childhood vaccinations wears out sooner than one thinks. The Tdap vaccination must be a family affair so that infants can be cocooned in a circle of protection. Vaccination is the single best way to prevent pertussis, so the Tdap vaccination is now recommended for adolescents 11 and 12 years old. Parents who have not been vaccinated are also urged to get Tdap, and now, even grandparents should roll up their sleeves. Tdap is now recommended for all adults, including those 65 and older, especially if they are around infants and young children. Note that the once-required interval between Tdap and the last tetanus booster has been removed, so there's no need to wait. The CDC also now recommends Tdap vaccination during pregnancy, preferably after 20 weeks' gestation, so that protective maternal antibodies pass on to the fetus.[4]
How well are we doing at getting people vaccinated with Tdap booster? The 2010 National Health Interview Survey released earlier this year found that about 69% of children 13-16 received the Tdap booster. Adult vaccination rates showed a small 1.6% increase in uptake, but overall, only 8.2% of adults have gotten Tdap.[5]
The next question that needs an answer is whether one booster for adolescents and adults is enough. But today, the more immediate question is, have you gotten your Tdap?
