September 26, 2008 — An infant diet that includes fish before the age of 9 months reduces the risk for the development of eczema, according to the results of a prospective, longitudinal study reported in the September 24 Online First issue of Archives of Disease in Childhood.

"The prevalence of eczema in infants has increased in western societies," write Bernt Alm, from the University of Gothenburg in Gothenburg, Sweden, and colleagues. "It has been suggested that environmental factors and the introduction of food affect the risk of eczema."

The objectives of this study were to determine the current prevalence of eczema among infants in western Sweden, to assess current patterns of food introduction into the infant diet, and to evaluate risk factors for eczema at age 1 year.

The study cohort consisted of infants born in 2003 in western Sweden. Half of the families of infants in the birth cohort (n = 8176) were randomly selected, and when the infants were age 6 months, the selected families were sent an invitation to participate, together with a questionnaire. When the infants were age 12 months, the families that had agreed to participate were sent another questionnaire.

Of the originally selected population, 60.2% of the families (n = 4921 infants) answered both questionnaires. Medical birth register data were also obtained for these infants.

Previous or current eczema was reported in 20.9% of the infants at age 1 year, with median age at onset of 4 months. A familial history of eczema, particularly in siblings (odds ratio [OR], 1.87; 95% confidence interval [CI], 1.50 - 2.33) or in the mother (OR, 1.4; 95% CI, 1.30 - 1.84), remained as an independent risk factor according to multivariable analysis.

Protective effects were noted for introducing fish into the diet before age 9 months (OR, 0.76; 95% CI, 0.62 - 0.94) and for having a bird in the home (OR, 0.35; 95% CI, 0.17 - 0.75). No associations with eczema were detected for short-term breast-feeding, the age at which milk or eggs were introduced, having a cat or dog in the home, or parental smoking.

Limitations of this study include possible recall bias inherent in questionnaire studies.

"Beneficial effects on infantile eczema were seen from introducing fish before nine months of age or having a bird in the home," the study authors write. "The duration of breast-feeding or the age at which milk or eggs were introduced did not affect the risk of eczema."
