【drug-news】Plavix 停药后心脏病发作风险可能再度提高

Heart Attack Risk May Rebound After Halting Plavix

Can the blood grow dangerously accustomed to a blood thinner? Perhaps, according to a study that found the risk of heart attack or death jumped when patients went off the popular blood thinner Plavix.

Researchers looked back at more than 3,000 patients who took Plavix after being treated for heart attack or unstable angina. The patients, who were discharged from VA hospitals between 2003 and 2005, took Plavix for about 10 months after leaving the hospital. The risk of heart attack and death was nearly twice as high in the 90 days after going off the drug relative to later months.

The authors say this could be a sort of “rebound” effect, and point to evidence from lab studies that suggests the blood may have a stronger-than-normal tendency to clot immediately after a patient goes off of an anti-clotting drug.

They also consider the possibility that the increased risk could simply be a function of still being at risk for another heart attack. But they discount this, noting that the risk increased after patients went off the drug whether they had been taking it for three, six, nine or more than nine months. They didn’t have enough patients who took Plavix for 12 or15 months — which has recently become more common — to see what the risks were in those groups, the Washington Post notes.

The authors conclude that the findings don’t necessarily offset the benefits of the drug, the WSJ notes. But they suggest it might be worth reconsidering the length of time patients take Plavix, and how they are taken off the drug (which is co-marketed by Sanofi-Aventis and Bristol-Myers Squibb).

Meanwhile, we’ll be curious to see what the profile looks like for prasugrel, Lilly’s new blood thinner that’s currently before the FDA.
